By creating an account, accessing, or using the Chart4Me platform (“Service”), you agree to be bound by these Terms of Service (“Terms”), our Privacy Policy, and all applicable laws.

Chart4Me is an AI-assisted clinical documentation tool designed to produce a sample documentation for licensed healthcare professionals and organizations.
The Service assists users in generating SOAP notes, nursing narratives, and functional capacity documentation using structured AI logic.
The Service is not a consumer medical app and is intended solely for professional use.

3. User Eligibility

Use of Chart4Me is restricted to:

Licensed nurses, therapists, or healthcare professionals;

Administrative personnel of healthcare organizations;

Users authorized by such organizations.

You must be at least 18 years old and capable of entering a binding contract.
By registering, you confirm that the information you provide is accurate and truthful.

4. Account Responsibilities

You are responsible for:

Maintaining the confidentiality of your login credentials;

Ensuring all activity under your account complies with these Terms;

Immediately notifying MG Clinical Solutions of any unauthorized access or breach.

MG Clinical Solutions  LLC  is not liable for any loss or damage resulting from unauthorized account access caused by your failure to secure credentials.

5. AI-Generated Documentation

Chart4Me’s AI tools assist with documentation generation based on structured inputs.
All AI outputs are drafts only and require professional review before being entered into any official patient record.
Users are solely responsible for verifying clinical accuracy and compliance.

The Service does not provide medical advice.

6. Subscription and Payment

Chart4Me operates on a subscription basis through Stripe, a PCI-compliant payment processor.
All billing, renewals, and cancellations are handled securely through Stripe.

By subscribing, you authorize recurring payments according to your selected plan until canceled.
Cancellation will prevent future billing but will not retroactively refund previous payments unless required by law.

All prices and plans are subject to change with reasonable prior notice.

7. Prohibited Uses

You may not:

Share or sublicense your account access;

Reverse-engineer, reproduce, or modify the software;

Use the Service for unlawful purposes;

Submit PHI (Protected Health Information) unless authorized under a HIPAA-compliant agreement;

Use the Service to generate misleading or fraudulent documentation.

Violation of these restrictions may result in immediate account termination.

8. Data Protection and Security

Chart4Me uses secure, encrypted infrastructure and does not store PHI unless specifically authorized under a written agreement.
All user information and payment data are processed in compliance with applicable privacy and security laws.

9. Termination

You may terminate your account at any time by contacting support or canceling via your Stripe subscription portal.

10. Intellectual Property

All code, logic, workflow design, graphics, and related materials within Chart4Me are the property of MG Clinical Solutions  LLC.
You are granted a limited, non-exclusive, non-transferable license to use the Service for lawful professional purposes.
All rights not expressly granted remain reserved.

11. Limitation of Liability

To the fullest extent permitted by law, MG Clinical Solutions  LLC , its officers, employees, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from or related to your use of the Service.
